Just to update you all on the situation, what brown suggested did work. Reading another post (here) I found that I needed to reset the state table (i.e close all connections and force them to reopen). I didn't do this at first, but now its working and i've had someone confirm its on the master server list.
So, in summary. If you run pfSense or m0n0wall (from which pfSense was forked) you need to enable advanced outbound NAT and set up a new rule above the default one so that in the end it looks like this:
