TF2 server crash

One of my TF2 server is crashing repetatively on class selection.
The error displayed on server console is:
./srcds_run: line 346: 5407 Segmentation fault $HL_CMD

The content of the debug.log is

I am not sure why it is consistently crashing. Please help me resolve the crash.

When I used the addtional command line option 2>debug.out... i got the following

Quote:./srcds_run: line 346: 21724 Segmentation fault (core dumped) $HL_CMD
cat: hlds.21708.pid: No such file or directory

What does this mean?
