What are your system specs/OS, and it is a dedicated server, vps, or local machine?

does it happen on certain occasions (like player join or map change)?

if it is not cause by plugins (make very sure you don't have any running, i.e. rename the addons directory and restart the server), it might be due to a broken installation. try updating with -verify_all. if that does not help, try reinstalling a fresh server without any changes (only passwords and server name! no maps, no settings, no plugins). if that works, apply all your changes one by one.

I found a solution:

1. Download an older version of vphysics.so:
http://tf2.ian-justman.com/fixes/

2. Replace your vphysics.so in the /yourserverdir/orangebox/bin (for TF2)

It solved the problem for me (I was having the "updating dump comment" crash on changemaps...)
