Remove them all and then redownload the lastest snapshot version of SourceMod and MetaMod: Source.
It's most likely caused by SourceMod.

But one of my servers is running without source mod and meta mod and does the same thing..

I updated SM, MM.. Crashed..
Then I disabled addons folder and started the server... That same problem.. Crashed on Swap team or typed in console "kill" ..

i disabled all my addons and it is still crashing... think it is a fail my be valve

Okay, I have just tested some things, and it seems like that the server crashes, when you SWAP teams.
There might be other reason, idk, but this is the ONLY thing, that I have found yet.
