HPET is on and BIOS power saving is off... still no dice. :/

Odd. Are the BIOS versions the same? Try running the game in RealTime mode (priority adjustment) to see if that helps.

I'm only using SourceMod & MetaMod, although this happens on servers without those as well.

Are those the only plugins listed?

Also, does there appear to be any errors in the game server console?
And compare the server configs between your game servers to make sure there are no errors / odd entries.

No errors in the server console. Configs are all the same.

Those are the only plugins listed.
