Issue: Server has been up and running for about two weeks, just lately (past two days) Almost everytime the server goes to another level, server crashes. I do have in-enabled RTV,and mapchooser.
Spec's: Windows 2003 x64 Standard Edition
AMD64 3200+ OC'd 2.21GHZ
2GB's DDR400
Start up config: srcds.exe -console -game zps +exec server.cfg +map zpo_biotec +maxplayers 14
Admin mod: Source Mod (newest release)

It's probably a compatibility issues with SM since that's the only thing that changed. Check for upgrades of any plugins you are using and try testing with SM disabled.
How are you changing the level?

If you disable the mods does it still crash?

Yes, when the RTV and map-chooser are turned off it doesn't crash the server. I'm gonna try and find updated versions of them or a fix for it.
