[REQ] Server Info/Players/Score via PHP or SQL

I am looking for a script that will output the players (actual list, not numbers of) in the server, players team, map it is on, and the score for T/CT into either something PHP can read or into a MySQL database.

I have seen plugins/scripts that do part of this, but none seem to list the team score, players names and players teams.

Is this possible, if so, can someone provide me with some help on getting started with score and player teams at least.
