I've been getting a ton of these as well on some Custom maps on TF2. Still trying to figure out what causes them.

Google tells me that a bunch of other people are having this issue.
No one has solved it yet.
I sent an email to Valve as well. Anyone have any luck contacting them?

EDIT: Forgot to mention that it happens with MM:S and SM disabled as well.
