If others are able to connect from outside the router to your static ip, it should be showing on the server's list. How are they finding your server to connect to it? Do you have any filters set on your steam server list search? (i.e. secure, region, map, etc....) I would try with no filters to search for. Your server should have a pretty low latency so it should be real close to the top. If it still isn't seeing your startup command line and server.cfg and port forward list would be helpful.
What type of router do you have? have you tried to update your routers firmware? also, on a windows box -autoupdate doesn't do anything, and has been known to cause wierd things to happen.. take it out, probly won't change anything, but its not doing anything anyways.
It's a netgear RP614. It has the "latest" firmware, dated Dec 2003 as shown on their site. I'm willing to try a 3rd party firmware if somebody can find one?

If this is true, the Windows tutorial on http://www.srcds.com shouldn't include it in the startup command line

In response netgear routers have worked really well with SRCDS in my usage, so do you also have a firewall on your computer? that may be also causing the issue if you do have a software firewall.
Problem is... the DSL modem is also a 1-port router. It was set up with DMZ to the Netgear router, and the Netgear was doing the port forwarding. In the config for the DSL modem/router, it was by default set to NAPT... I set it to NAT and voila!
