in your server.cfg make sure you have:

sv_lan 0

also if your server is behind a router, make sure you have port 27015 forwarded to your server or people will be unable to connect after a while (routers will close inactive open ports after a set time). for instructions relevant to your specific router, see http://www.portforward.com.
