have you disabled cool and quiet? Try this as root:
cpufreq-set -g performance

The standart kernel has cpuutils build in. So either rebuild it without them, or set them to performance mode.
Your drops happen because the cpu can not scale up the cpu frequenzy fast enough for the load peaks of the source server.

cpufreq-set -g performance
-bash: cpufreq-set: command not found

Perhaps an older xeon that cant hold the load?

OS: Debian kernel 2.6.26.8-rt16 <<< that looks for the tut of fragholic. normally he should switch off cpufreq. But i dont think that these are real lags. Its to much for the CPU and engine with a lot of players and plugins you cant turn of them all (drops). but i think you dont feel anything ingame.

there are many drops below the tickrate, I am pretty sure that one will feel those lags and they are very real. and:

so I guess, those are real lags...

@TO:
What kind of Xeon CPU is this? can you post the output of the following command:
cat /proc/cpuinfo

Did you really follow my howto? If so: Did you do all checks I wrote in the "trouble shooting" section?

but normally ... if there are only lags noone would play on this server > but you can see there are a lot of players. a lot of gamers watching to much on fpsmeter and think the server is lagging althought it could be an other problem > normally to mani plugin ... old plugins not updatet .... eventscript e.g.
