I don't observe something like this, on the contrary. Since the update the hit registration has even improved on my servers (the tiny bit it could get improved... ;-)). Ok, I am on Linux, not Windows, and I did a lot of system optimizations, but that did not need any changes. Your rate settings are fine. I would recommend using the valve default settings for all the other stuff. And I recommend fps_max 70 to keep the server running at 66 fps, which is the optimum for the orange box (for tick 66 servers).
